• Call - (512) 305-8530 to request a complaint form to be sent to you.
• Mail - Request a form be sent to you by writing the address below.
• Fax - Request a form be sent to you by faxing us at (512) 305-8900 c/o Investigations.
• If you request a form be sent to you, we can send it to you in various formats (email, fax, mail), but we ask that you mail your complaint so that we have your original signature and the originals of any evidentiary supporting documentation.
