Reinstatement of a Voluntary Surrendered License
A license that has been voluntarily surrendered may qualify for reinstatement within five years of the date of surrender. After five years, an individual seeking reinstatement must first be reexamined or furnish evidence of registration in another jurisdiction.

Reinstatement of a Revoked License
A license revoked for any cause may be reinstated only by a Board action. A request for reinstatement must be addressed to the Executive Director and explain why a reinstatement is justified.
